Preparation of Central Venous Catheter for Initiation
and Discontinuation of Hemodialysis Treatment

Purpose:
To prevent contamination and infection of the dialysis catheter access during initiation and discontinuation of the hemodialysis treatment.

Supplies:
Alcavis 50 solution
, 2 packages of sterile 4x4 gauze pads, chux sheet, sterile underpad, non sterile gloves, staff face shield, patient mask.

Procedure:

  • Staff should wash their hands and don a face shield. Patient should don a face mask.
  • A sterile underpad should be placed under the catheter.
  • On a chux sheet, carefully open both packages of 4x4 gauze pads keeping pads on sterile wrappers.
  • Saturate both sets of 4x4 gauze pads with ample amounts of Alcavis 50 solution (10 to 15 cc's for each gauze pad).
  • Put on clean non sterile gloves.
  • Carefully place the catheter venous port in an Alcavis 50 saturated 4x4 gauze pad and scrub the catheter end and port for 1 minute.
  • Carefully place the catheter arterial port in an Alcavis 50 saturated 4x4 gauze pad and scrub the catheter end and port for 1 minute. Make sure to rub in an agitating motion when cleaning ports.
  • Carefully wrap the arterial port in an Alcavis 50 saturated gauze pad.
  • Carefully wrap the venous port in an Alcavis 50 saturated gauze pad. Leave each port wrapped for at least 2 minutes.
  • Carefully unwrap the catheter ports for initiation of dialysis treatment as per clinic protocol.
  • Repeat this process for the discontinuation of the dialysis treatment as per clinic protocol.

Amu-Safe Procedure


Purpose:
To disinfect dialysis disposables.

Supplies:
Alcavis 50
, PPE, non-sterile gloves.

Procedure:

  • After completion of C.A.P.D., exchange procedure. Leave Alcavis 50 in upright position, twist luer connector of the Y-set onto Alcavis 50.
  • Turn Alcavis 50 over and elevate bottle to eye level.
  • Visually locate measurements on side of bottle.
  • Unclamp outlet port clamp on the drain line.
  • Squeeze bottle until you have infused 25ml.
  • Clamp outlet port clamp on drain line.
  • Place bottle in upright position.
  • Disconnect bottle, cap bottle and store from sunlight.
  • Wait 5 minutes before disposing of solution and tubing.

Transfer Set Change using Alcavis 50

Purpose:
To change the transfer set using a sterile technique.

Supplies:
Alcavis 50
, sterile gloves, 3 packages of sterile 3x3 gauze pads, 2 sterile drapes, plastic clamp without teeth or seam, transfer set and other supplies to complete exchange, 2 masks.

Procedure:

  • Have patient remove transfer set from clothing.
  • Place plastic clamp on the peritoneal catheter.
  • Staff should wash their hands and don a face shield. Patient should don a face mask.
  • Open sterile drape.
  • Open all packages of sterile 3x3's. Saturate two packages of 3x3's with Alcavis 50 (at least 20 cc's for each gauze pad).
  • Have patient pick up catheter and transfer set.
  • Nurse needs to place the sterile drape under the catheter and transfer set, and have patient drop transfer set onto the drape.
  • Prepare transfer set to be attached at this time.
  • Put on sterile gloves.
  • Pick up the first set of 3x3's saturated with Alcavis 50, and scrub at connection between transfer set and patient catheter, maintaining contact for one minute.
  • Using the second set of Alcavis 50 saturated 3x3's, scrub the connection site for another minute, maintaining contact time.
  • Pick up dry 3x3's, and drop patient transfer set and catheter into dry 3x3.
  • Using twisting motion, disconnect patient transfer set.
  • Pick up new transfer set, maintaining sterility, and apply new transfer set to catheter making sure connection is tight.
  • Discard used supplies and proceed with exchange.

Procedure for Disinfecting Minicap Transfer Set after Contamination


Purpose:
To disinfect the end of the transfer set after a touch contamination.

Supplies:
Sterile specimen cup, bottle of Alcavis 50, mask, sterile minicap.

Procedure:

  • Wash hands.
  • Using sterile procedure, open sterile cup.
  • Open minicap.
  • Fill cup 1/2 -3/4 with Alcavis 50.
  • Place end of the transfer set, into Alcavis 50, and soak for 1 minute.
  • Remove transfer set. Using sterile procedure apply new minicap.
  • Teach patient to observe patient for signs and symptoms of peritonitis and to call unit if symptoms develop.

Procedure for Having Patient Disinfect Minicap Transfer After Contamination at Home

Purpose:
To disinfect the end of the transfer set after a touch contamination at home.

Supplies to give to patient:
3 sterile specimen cups (extras to keep at home), bottle of Alcavis 50, mask, sterile minicap.

Procedure:

  • Wash hands.
  • Using sterile procedure, open sterile cup.
  • Open minicap.
  • Fill cup 1/2 -3/4 with Alcavis 50.
  • Place end of the transfer set, into Alcavis 50, and soak for 1 minute.
  • Remove transfer set. Using sterile procedure apply new minicap.
  • Teach patient to observe for signs and symptoms of peritonitis and to call unit if symptoms develop.
  • Teach patient to record symptoms, on his/her logs when they need to, and to call the unit or nurse on call if they have any problems.

Adding Intraperitoneal Medication to Dianeal

Purpose:
To add prescribed medication to dianeal in a sterile manner.

Supplies:
Appropriate dianeal solution, Alcavis 50, syringes (number depends on volume of medication to be given), sterile needles (22g. 1/12in.), appropriate medications, mask, labels, tape, 2x2 gauze pads (2 packages).

Procedure:

  • Don Mask.
  • Check medication bottle or prefilled syringe for correct type, dose and expiration date.
  • Aseptically place Alcavis 50 soaked 2x2's on rubber port of medication vial and dianeal.
  • Leave Alcavis 50 soaked 2x2's in place for two minutes. Open syringe and aseptically attach sterile needle to syringe. Draw up prescribed amount of medication. Recheck amount of medication.
  • Recap needle loosely on syringe and lay on dominant side. Remove Alcavis 50 2x2's from med port and position in non-dominant hand without contaminating.
  • Retrieve syringe in dominant hand and withdraw cap.
  • Insert needle through medication port of dianeal bag and inject medications. Aspirate dialysate into syringe and reinstall.
  • Mix medication thoroughly by inverting bag several times.
  • Label bag with medication, dosage, time, date, and nurse's initials.
